With families growing farther apart and seniors often living alone, it’s important to have a care management team you can rely on. Silver Wings care managers are experienced in working with aging adults and their loved ones to assist with remote coordination of medical appointments, living arrangements, socialization, and other needs. A lot of our clients refer to their care manager as their “rented daughter with expertise.” Our goal is to provide our customers with the best aging care management services in the industry and help them get back to what matters most, their family and lifestyle.

Care management programs offer comprehensive support for members with complex healthcare needs and high risk health conditions. Our RN and LPN care coordinators are able to coordinate a member’s individualized health plan, which is based on their unique needs, preferences and medical history. They also provide a patient’s physician and other healthcare providers with a summary of the member’s medical history, including a list of medications and current health concerns.

During the initial contact, members will be assigned a lay navigator. They will help to complete the required paperwork, review medication lists and allergies, and update general demographic information. They will also introduce you to your RN Care Manager, who you will be able to communicate with in the future.

Our RN care management team is comprised of experienced geriatric care professionals with a passion for helping seniors and their families. Our geriatric care management services are designed to improve outcomes and reduce hospitalizations by identifying gaps in healthcare and offering support to manage the member’s chronic health conditions and behavioral health needs. We also work with our partners to ensure a positive member transition from acute care and into post-acute/long term care recovery. This is accomplished through a suite of AI-powered, predictive tools that provide early insights into member health changes and identify critical performance gaps before they become a problem. Our RNs are certified by the Aging Life Care Association (ALCA) as Geriatric Care Managers and adhere to their strict code of ethics and professional standards.

As the founder of Silver Wings, Erin knows first hand the challenges families face when caring for aging parents who live far away. She has also served on the board of the Mid-Atlantic Chapter of ALCA and is a Certified Geriatric Manager and a CMC.
